Rear Drum wheel bearings loosen after ride

I have a 5 spd rear freewheel with a drum brake.My problem is after a few miles the bearings start to loosen.I have tightened everything and it is still happening.Is the drum brake arm causing this?? Some how ??? Any comments or suggestions would be great.

It is a Rotatex hub..I've seen these on a few of the Husky wheels also.
