chopper brakes?

well ive got about 58 miles on my chopper and so far my stock rear brakes have worked even goin down a big hill i live on, but i want to know what i need for a disk brake on my front wheel. what do i need?

A fork which supports disc brakes (or an adapter, I suggest a compatable fork though)
A wheel hub which supports discs.

Notice the 5 screws holes? (there's another one, the camera angle hides it though), the disc screws on here.

thanks um do they have any forks that resemble a chopper fork? ive been looking i cant find on ..is there anyway i can customize the orignal?
