For a motorized bicycle, you're going to need to come up with a title. To get a title, you need a vin. For that, you will need to go through the enhanced inspection process. Before that inspection, you will need to equip the bike exactly as a moped would be equipped.

To expound on what PAracer said, in order to pass the enhanced inspection (which costs about $160 and up), you will need DOT-approved headlight, tail light and tires. A brake light and directionals are not required, but should you choose to install them I recommend that they also be DOT approved or you wait to install them till after you pass the inspection. You will need to have the enhanced inspection form notarized, apply for a vin and title (bicycle serial number can be used as vin), and then apply for a plate. You will also be required to have a driver's license and insurance.
