Default replacing rear wheel bearings

After riding my bike a little while, the rear wheel bearings went.. they were weak to start with, having a little play.. I did grease them but they didn't look nice then, and certinly not nice now...

problem is, I'v never replaced the bearings.. how is this done?... I can do them on cars ( sealed and non sealed ) just never a bike

Thanks for the link.. looks like I may just take it to a bike shop. I don't have the cone wrench to get it apart, and I don't feel the cost of those tools and buying the parts will be worth it, when probably for $15-20 I can have the bike shop do it

dropped it off at the shop.. turns out the entire shaft has to be replaced..
