chain problems

well first almost heard the monster go and my chain popped of i welded part of the tesioner to the bike but not the wheel so chain was tight and the wheel was not leaning to the center but after looking at my sprocket it looked off but not the wheel as my homade tensioner is like 4 inches long and its an old school ten speed so i didnt mind welding on it. was thinking it was the sprocket as it always popped off even with my other ten speed same problem with the stock sprocket. well any ideas of my problem shoot at me cause my bikes will be fixed soon so I want to ride my scoot. And i had a gas leak from carb or carb line gonna go look tommorow.

If the sprocket isn't perfectly centered on the wheel or it wobbles from side to side when you spin the wheel, that can cause the chain to come off.
Also, the tensioner must feed the chain straight onto the sprocket.
