New 36 teeth sprocket falling off..?

So here recently ive revived my old beast tuned it up again and put my 36t sprocket ive been waiting to use for almost a year...I finally got it on and took her for a run and it did pretty damn good! But whenever I get up in my higher RPM's and speed for some reason the chain comes off gets stuck around the bolt and the engine dies...ive been using a modified version of the stock tensionor I made into a spring tensionor..ive been using it for a long time now and ive got no idea whats going on with this..my old 44t was dished and this new one is flat..any help? THANKS!!.shft.

Check your Chain alignment and ajustment. Chain adjustment should be 1/2" to 3/4" play without depending on your modified chan tensioner. Make sure your sprocket is running true.

also, bend or add washers to your tensioner so that it feeds chain exactly straight onto bottom of rear sprocket

were it I, a solid tensioner would be on there

Hey thanks guys..put some rubber on the tensionor to hold it down and straighten things out...got up to 40 today! Love it!!
