What I did with my grip shifter on the bars was simply move it over to the left side of the bars. Eventually, I also cut down the left side grip, so that I could still grip shift, instead of having to reach over the lip of the handlebar grip that came with the kit. It's easy, functional, and looks real nice.

Eliminating the rag joint - apart from the aforementioned shift kit, there are a number of outlets that sell a variety of hub adapters for the rear sprocket. Pirate Cycles has a kind that clamps on the hub, there are others like that, too. Spookytooth has a kind that grabs the spokes, but it's all metal instead of rubber.
